At the six-week check they tell you you're cleared. Cleared to exercise, cleared for sex, cleared — and this is where it gets strange — to start trying again, if that's what you want. Six weeks. Meanwhile the baby still wakes twice a night and you can't remember the last meal you ate sitting down.
That six-week number is the most misread figure in maternity care, and almost every question about postpartum fertility collides with it first. It was never a statement about postpartum fertility at all. It's a rough estimate of how long the uterus takes to shrink back and the cervix to close — a wound-healing timeline that got quietly promoted into life advice.
The real question, the one you're actually asking, is whether your body is in a position to carry another pregnancy well. That has several answers. They don't fully agree with each other, and I'd rather show you the disagreement than smooth it over.
So this is the honest version of postpartum fertility: what the guidelines say, where they contradict each other, what your own body is doing about it without consulting you, and the two cheap tests almost nobody offers you first.
What the guidelines say about postpartum fertility, and how much they disagree
In 2005 the World Health Organization convened a technical consultation on birth spacing in Geneva. The recommendation that came out of it, and that has shaped postpartum fertility advice ever since: wait at least 24 months before attempting the next pregnancy, which works out to about 33 months between births. After a pregnancy loss, at least six months.
Then, in 2018, a large cohort study in JAMA Internal Medicine complicated the postpartum fertility picture considerably. Laura Schummers and colleagues looked at 148,544 pregnancies in British Columbia and did something the earlier work mostly hadn't — they split the results by maternal age. What they found runs opposite to what most people assume.
Compare a six-month gap against an eighteen-month one. For women 35 and over, the risk of maternal death or severe illness went from 0.26% to 0.62%, an adjusted relative risk of 2.39. For women aged 20 to 34, that same short gap produced no increase at all — 0.23% against 0.25%, an adjusted relative risk of 0.92. Now flip to the babies. The younger group carried the larger share: an adjusted relative risk of 1.42 for the adverse infant outcome, against 1.15 in the older group. Spontaneous preterm delivery followed the same shape, 5.3% versus 3.2% among the younger women.
Read that twice, because the popular version of postpartum fertility risk has it backwards. Older mothers carried more of the risk to themselves. Younger mothers carried more of the risk to the baby. The authors' own conclusion stays deliberately broad: "Short interpregnancy intervals are associated with increased risks for adverse pregnancy outcomes for women of all ages."

A 2023 analysis in The Lancet Global Health put a much bigger dataset behind the same question — 692,402 pregnancies drawn from 113 Demographic and Health Surveys across 46 countries. Against the WHO's recommended window, the hazard of perinatal death was 2.72 times higher when the gap was under six months. At six to eleven months it fell to 1.64. At twelve to seventeen months, 1.15. At twenty-four to fifty-nine months, 1.00 — flat.
Which tells you something practical about postpartum fertility planning that the round number hides. The risk is concentrated at the very short end. Under six months is a real signal. Twenty-four months is a target to aim at, not a wall that falls down at twenty-three.
The postpartum fertility rule that just got overturned
The same 2023 paper tested the WHO's second recommendation, the six-month wait after a loss. It didn't survive. The authors write that intervals "of less than 3 months, 6 months, and 12 months following stillbirth or abortion (spontaneous or induced) do not pose a higher risk for perinatal death in subsequent pregnancy," and state plainly: "We found no beneficial effect of the WHO recommended interval of 6 months from a pregnancy loss to the next pregnancy."
If you've been told to wait half a year after a miscarriage before thinking about postpartum fertility again, that instruction now has a very large dataset arguing with it. Worth raising, if it's shaping your decision.
And here's the part I'm not going to leave out. A commentary published in the same issue took the study apart: it runs on cross-sectional survey data, so recall bias is live, the findings are correlations rather than causes, and the countries involved changed enormously across the two decades covered. Their verdict — "it might be premature to regard existing WHO recommendations as ineffective in all contexts." Both papers, same journal, same month. That's what a question still being argued actually looks like, and anyone selling you a clean answer on this is selling you the confidence, not the evidence.
Why postpartum fertility doesn't come back on a schedule
While you're reading guidelines about how long to wait, your body may have already decided for you. Postpartum fertility isn't sitting idle waiting for your permission — it's being actively held down, and there's a mechanism doing the holding.
Breastfeeding suppresses ovulation, and the pathway is well mapped. Suckling drives prolactin up and keeps it there. A 2022 review in the International Journal of Molecular Sciences puts the next step precisely: "Hyperprolactinemia induces the suppression of hypothalamic Kiss1 neurons that directly control the pulsatile release of GnRH." Those kisspeptin neurons set the rhythm of GnRH release, and when the rhythm breaks, so does everything downstream — "disruption in the pulsatile manner of GnRH secretion results in a strongly decreased frequency of corresponding LH pulses," which stalls the follicular phase and produces anovulation.
The same review gives a rough threshold for how much feeding keeps postpartum fertility suppressed: at least five times a day, with total duration above 65 minutes, tends to hold amenorrhoea in place.

So here's the practical payoff, and it's the most useful thing on this page. Four months of trying with nothing happening, while you're still feeding a baby several times a day and through the night, is not a fertility problem. It's the mechanism doing exactly what it does. Postpartum fertility returns on a schedule that's partly set by how often that baby feeds, and nobody tells you that before you start counting months and worrying.
Two honest limits on it. The same authors admit "the exact mechanism explaining how the frequency and duration of suckling lead to the suppression of LH pulses is still not exactly explained" — the pathway is clear, the dose-response isn't. And ovulation comes back before the first period does, so postpartum fertility can return with no announcement at all. The mechanism is real. It is not a calendar, and it's certainly not a contraceptive you'd want to bet on.
There's a well-known figure quoted everywhere for how protective full breastfeeding is in the first six months. I went looking for it at the primary source and hit a bot wall I wasn't willing to work around, so I'm not going to hand you the number. It would have made this section tidier. That's usually the reason to be suspicious of a tidy number.
Two things worth testing before you call it a postpartum fertility problem
Iron, and the normal blood count that isn't
A Norwegian cohort published in the Journal of Nutritional Science in 2022 measured 573 women at a mean of 13.9 weeks after birth. A quarter of them, 25%, were anaemic. But 39% were iron-deficient by serum ferritin.
That gap is the whole point. More women had empty iron stores than had a low haemoglobin, which means you can walk out of an appointment with a normal blood count and still be running on nothing. If someone checked "your bloods" and said they looked fine, it's worth asking whether ferritin was among them. Often it isn't. And depleted iron is not a small footnote to postpartum fertility — it's fatigue, poor recovery and a body with no reserve, at exactly the moment you're asking it for more.
Hold the numbers at their real size: one cohort, one country, one timepoint, 573 women. A signal worth acting on, not a national statistic. The authors also note that their three different measures identified meaningfully different groups of women, which tells you the thresholds themselves are still contested.
Thyroid, and the postpartum fertility problem that gets called exhaustion
Postpartum thyroiditis turns up in roughly one in twenty women after birth, somewhere in the 5 to 8% range, with rates running from 1.1% in Thailand to 13.3% in Brazil. It usually arrives in two phases: an overactive stretch between one and four months postpartum, then an underactive one, usually around four to eight months. Between 20% and 50% are still hypothyroid at a year, and up to a fifth end up needing treatment for life.
Now look at the symptom list. Tired. Low mood. Weight that won't behave. Anxious, then flat. Every one of those is also a symptom of having a small child, which is precisely why it slips past — the clinical literature says outright that because these overlap with "typical postpartum changes, such as fatigue, mood swings, and weight fluctuations," the condition is "often underrecognized and misattributed to other causes."
A thyroid panel is a cheap test. If postpartum fertility is the thing you're now planning around, it belongs on the list before anything more elaborate does. Ask for it by name.
The nervous system part, and where it stops
You already know what I do for a living, so let me be careful here rather than convenient.
A body six months into broken sleep is not in a neutral state. It's running a night shift it never agreed to, listening for a sound in the dark even on the nights nobody cries. That's not a mood. It's a nervous system holding a posture, the same bracing I've spent years watching people carry into rooms without knowing they're carrying it.
What I won't do is tell you that stress delayed your postpartum fertility, or that calm will restore it. The evidence doesn't carry that weight, and this series has said so before. The narrower claim is the honest one: a nervous system that hasn't had an unbroken stretch of sleep in months can't be talked into readiness with a breathing exercise. It needs the thing it's actually missing. Usually that's sleep and hands — a logistics problem far more often than a spiritual one.
That matters for how you read your own postpartum fertility, because exhaustion invites the wrong conclusion. A body this depleted looks, from the inside, like a body that's failing. It isn't. It's a body that has been allocating everything it has to keeping something else alive, and has been doing it well.

Which points somewhere unglamorous. Protect one block of sleep instead of trying to fix all of it: four hours that somebody else covers, same nights each week, defended like an appointment. Ten minutes of practice rather than thirty, because thirty is how people quit. Eat something with protein in it that you didn't have to cook. None of that will move a ferritin level or a thyroid antibody, and none of it is meant to. It keeps the system from bracing harder while the slower things get sorted.
When to stop waiting and ask someone
None of this is a diagnosis, and none of it replaces the person who knows your history. It's a list of things worth asking for, which is a different thing and often harder to get.
If your periods haven't come back within about three months of properly weaning, that's a conversation. If you had a stretch of racing heart and heat intolerance and then, a few months on, the opposite, ask for a thyroid panel specifically. If exhaustion is out of proportion to the sleep you're getting, ask for ferritin rather than a general blood count. If your mood is flattening rather than swinging, that's worth naming out loud to someone, quickly. And the usual threshold still holds: twelve months of trying without success, six if you're over 35, and both partners get looked at. Both. Not you alone.
The six-week clearance answered a question about tissue. It was a decent answer to that question and a terrible answer to yours. What settles postpartum fertility is iron, thyroid, how often a baby feeds, how much sleep gets returned to you, and time — most of which doesn't appear on any calendar and none of which is a verdict on you.
If you take one thing from all of this, take the reframe. Postpartum fertility that hasn't come back yet is not postpartum fertility that has failed. It's a system that made a call about timing while you were busy, and it made that call using information — feeding frequency, iron, sleep, thyroid — rather than judgement.
Your body isn't behind schedule. It's running one you didn't write.
